Overemphasis on low purine diet is detrimental to prevention and treatment of gout because:
1) About 80-98% of people who have high blood uric acid (UA) levels (hyperuricemia) do not have gout in their life time.
2) Strick low purine diet can reduce the blood UA level only about 1 mg/dL which is rather small in helping gout. Modern medical & dietary advice no longer insist on strict low purine diet for gout patients.
3) Sudden lowering of the blood UA level can trigger gout, e.g., initialtion of Allopurinol triggers gout because it reduces the blood UA level suddenly.
4) Strict low purine diet often causes physical and mental stress on gout patients. It is well known that stress is a strong gout trigger.
5) Most of all, it detracts gout patients from paying attention to other more useful things they can do for gout.
For example, the following items involve no or very little purines intake, but they can trigger gout. What do they in common? They lower pH in the body and the gouty joints:
* Cranberry juice (ph=2.3~2.5), tomatoes (ph=4~5), dill pickles (ph=3.2~3.7), and carbonated drinks (ph=2~3) -- they are low in ph.
* Aspirin (acetylsalicylic acid), niacin (nicotinic acid) and Vitamin C (ascorbic acid) -- they are all acids.
* Vigorous exercise -- it lowers the ph at the joints due to the extra production of lactic acid.
* Type 2 diabetes -- due to the lack of insulin, the body is unable to burn glucose for fuel. As the consequence, the body burns fat for fuel and produces excess amount of acidic ketones.
* Starvation and rapid weight loss -- the body burns excess amount of fat for fuel and produces too much acidic ketones.
* Injuries, osteoarthritis, rheumatoid arthritis, tenosynovitis, etc. -- they cause the deterioration of tissues at the joints and lowers their ph.
* Emotional stress, serious illness, hospitalization and surgery -- the unhealthy states that degrade the joint fluids and lower their ph.
* Sleep -- gout attacks occur most often at night (12-2 a.M.) because the stomach stops producing juice in sleep and causes acid tide in the body.
